Service Change Request Forms
In order to provide timely service, please use these forms to request changes to
your telephone key designation labels, telephone system speed dials, voicemail auto
attendant greetings and telecom services. Please download and complete the appropriate
form with the requested changes. Then submit your service request by calling 1-800-767-4991,
option 2. At that time you will be instructed to email or fax your documentation
to our service department.
